Since the last version I've done a lot of work to improve Menu Filter for the menus attached to toolbar/panel buttons - history, bookmarks, and developer tools. The help menu on the panel (a.k.a. hamburger menu) is now filtered to match the help menu on the menu bar, as requested by a user.

This version attempts to improve what happens when items are removed:
* After items are hidden, if no items remain between two separators, one is removed. Similarly, if a separator is the first or last visible item, it is removed.
* In Thunderbird, if an item is hidden from one of the main menus, Menu Filter attempts to hide the corresponding item on the hamburger menu also.

You can also now hide the 'Open All in Tabs' item from bookmarks menus, and a bug has been fixed where some items weren't shown on the configuration page.
